SALARY: $32.11 to $34.04 per hour

As part of the Community Strategies and Family Supports Department, the Community Resource Navigator will be an integral part of the Community Resource Navigator Program, helping individuals and families to access community resources including health care, employment services, housing options, child care and other needed supports. This is an in-person/on-site position, based out of the Ontario Works Office in Brantford.

Duties include but are not limited to:

  • Participate as a team member on the Community Resource Navigation Team
  • Provides navigation services to Ontario Works clients, who may include individuals living with complex needs such as mental illness, addiction, and/or experiencing homelessness
  • Support successful referrals to service providers in the community as necessary
  • Support clients to identify and attend appointments, assessments, and programs to support employment and well-being goals
  • Collaborates with internal and external partners and staff to ensure the effective and efficient delivery and training related to designated services
  • Seeks out resources and partnerships that will benefit underserved and/or marginalized families
  • Other duties as assigned

QUALIFICATIONS

  • Candidate must possess a minimum one (1) year public post-secondary diploma/certificate in Community or Social Services or a related field from a recognized public post-secondary institution.
  • Minimum one (1) year of relevant experience working with at risk and/or marginalized communities, including complex social issues such as poverty, mental illness, addiction, housing issues/homelessness, illiteracy, health issues, and concurrent disorders.
  • Familiarity with municipal policies and procedures, as well as local community agencies and service providers, is considered an asset
  • Working knowledge of pertinent provincial and federal legislation and guidelines related to mental health, income support programs, employment and training such as Housing Services Act, the Residential Tenancies Act, Child Welfare Act, social assistance legislation (ex. Ontario Works and ODSP), Municipal Freedom of Information and Protection of Privacy Act considered an asset
  • Ability to travel to other locations in the community, as needed
  • Satisfactory Police Check Vulnerable Sector required as a condition of employment

WAGE/SALARY RANGE: $32.11 to $34.04 per hour
